Checklist for Construction-Related Companies
For companies seeking to do construction, including roads, bridges, waste and drinking water systems, and energy conservation/efficiency-related construction/renovations, below is a list of documentation you should be prepared to provide:

Company information
- Short history of each company
- Tax ID number for each company
- List of Officers - Full name (including middle initial), address, and social security number, if applicable
- For LLC, provide articles of organization, operating agreement, and names of members
- Any relevant licenses/certifications, including but not limited to contractor's license
- Contractor's license number, if applicable
- Property and Casualty Insurance information - Certificate of Insurance
- Workers' compensation insurance verification
- Surety bond - additional information on LED's Bonding Assistance Program at http://www.ledlouisiana.com/opportunities/incentives--programs/bonding-assistance-program.aspx
- Brief resumes of key personnel, to include superintendents and foremen
- Past three (3) fiscal year-end financial statements:
- Preferably prepared by a CPA on a review or better basis;
- Use the percentage of completion method of income recognition
- Include all appropriate notes, schedules, etc.
- Concurrently dated personal financial statement(s) for the principal owner(s) of the business.
- Bank reference letter:
- Should be dated after last fiscal year end date
- Should specify dollar amount of line and how line is secured
- Should provide line-of-credit, including current amount of line in use
- Miscellaneous items:
- Names and addresses of suppliers/creditors
- Names and addresses of references for previously completed jobs
- Letters of recommendation/accommodation on completed projects
- Work in Progress Schedule
